What makes Fotona 4D genuinely different is its inside-out approach. Most lasers treat the skin surface. Fotona 4D starts inside the oral cavity, heating the deep connective tissue below the jawline, then works outward through the dermis before finishing at the skin surface. The result is full-thickness collagen remodelling that no single-mode treatment can replicate.

Inside the oral cavity
The treatment starts inside the mouth. Er:YAG laser energy is applied to the mucosa, generating controlled bulk heating in the submucosal tissue directly beneath the skin surface, the same tissue responsible for jawline definition and nasolabial fold depth.

The heat triggers immediate collagen fibre contraction and stimulates new collagen production, lifting and volumising the jawline, cheeks, and perioral region naturally, mimicking the effect of filler without injectables.

FRAC3 uses the Nd:YAG laser in a fractional pattern. The beam is whole upon delivery to the skin surface, then fractionated by imperfections as it penetrates deeper layers. This means energy is concentrated precisely where the skin’s structural problems exist, without damaging healthy surrounding tissue.

The result is selective rejuvenation of deep lines, pigmentation, and areas of volume depletion, improving skin density and texture where structural aging begins.

PIANO uses a super-extended Nd:YAG pulse, seconds long rather than milliseconds, combined with a gentle brushing technique across the face. This concentrates energy subcutaneously, creating safe rapid bulk heating through the full thickness of the dermis while the epidermis remains protected by the continuous motion.

This deep thermal effect triggers a broad tightening response, firming and lifting facial tissue globally rather than spot-treating individual lines or areas.

The final step is a precisely controlled light peel using Er:YAG in Fotona’s proprietary VSP mode. Unlike conventional peels, VSP delivers cold ablation, removing the outermost skin layer without thermal side effects, meaning the epidermis is resurfaced without inflammation or extended recovery.

Starting inside the oral cavity with SmoothLiftin™, then progressing outward through FRAC3®, PIANO®, and finally SupErficial™ at the skin surface. This full-thickness approach stimulates collagen at every level, something no other single treatment achieves in one session.
Most patients describe the sensation as a comfortable warmth, similar to a hot stone massage across the face. The intraoral phase is usually the most novel sensation but is not painful. No anaesthesia is required, though numbing cream is available if preferred.
For optimal results, 3 to 4 sessions spaced about 4 weeks apart are recommended. Many patients notice immediate tightening and glow after their first session, with progressive improvement over the following months as collagen production increases.
No. You can return to daily activities immediately after treatment. Some mild redness or flushing is normal and typically subsides within 30 minutes to a few hours.
Results typically last 12 to 18 months. Because the treatment stimulates your own collagen production, results continue to improve for up to 6 months before gradually fading as natural aging continues.

Avoid sun exposure and tanning for at least one week before your session. Discontinue retinoids and exfoliating acids a few days prior. Arrive with clean, product-free skin.
For mild to moderate volume loss, Fotona 4D can achieve comparable volumisation naturally without injections. For significant structural volume deficit, a combination approach may still be recommended.
